OBIETTIVI:
1)To accurately define a calibration protocol for the visualization of the coil position relative to the brain durino the stimulation in real time by frameless stereotaxy.
2)To accurately segment from the morphological MRI acquisitions the skin as well as the main structures of interest, i.e. the skull, the cortical and subcortical grey matter, the white matter, the cerebrospinal fluid and the lesion.
3)To develop an accurate model-based deformable registration method to align the MR images of brain tumor patients to a reference normal brain atlas.
4)To accurately plan the surgical resection of brain tumors by identifying the regions of activation during the TMS and evaluating the behaviour of the system in presence of infiltrative or not infiltrative lesions.
5)To evaluate the correlation between the functional activity areas as highlighted during an fMRI scan and those areas stimulated by the optical-tracked TMS system.
ABSTRACT: L’esigenza di preservare la funzione corticale di pazienti affetti da tumori cerebrali ha stimolato il potenziamento del monitoraggio prechirurgico della funzionalità della corteggia motoria e la diffusione dei sistemi di monitoraggio intraoperatorio. Inoltre la crescente esigenza di standardizzazione delle procedure seguite da diversi centri ha sollecitato l’adozione di sistemi di neuronavigazione che garantiscano una maggiore accuratezza, riproducibilità e robustezza nell’attività clinica principalmente a causa della complessità computazionale e della difficoltà d’uso. L’obiettivo di questo progetto consiste nel progettare e realizzare un sistema di neuronavigazione che consenta di oltrepassare tali limiti e fornire una soluzione di avanguardia nel settore.
